Bizarre/Suicidal Clown Death

I'm having the worst luck with clowns.
I have a new pair from Jason & Betty that have been doing great - woke up this morning to find one that perished over night.
Have them in a JBJ Nano with a modified output splitter with an opening of just over an 1/8" and one of the fish decided to try and swim into it last night.
Looking at the pictures it's actually rather bizarre as the fish would have needed to force itself into the small opening - it went about 1/3 of it's body length.  These are outputs (not inputs) so I can't really imagine what happened.  It was wedged so tightly that I was afraid it wouldn't come out in one piece:

Was it possible something was chasing it, and it was looking for any opening possible?

I have seen my clowns in front of the outlet alot almost like they are riding a wave.

Possible but not likely - this is mostly a bare tank (aside from the zoas in my other post that I'm trying to keep alive) with only the 2 baby clowns, a hermit and a pair of small chromis in QT.  None of the fish are established (since last Saturday) so I can't imagine aggression yet.

Maybe it just thought it was a good hiding place since it was dark and got stuck? My female black saddleback clown shoots herself like a bullet inside her anemone when she is scared. All you can see is a tail sticking out. The first time I found her that way I thought she was dead and the anemone was trying to eat her. I have seen some of my fish playing with the outflows as well. Maybe they like the feel of the flow against their skin? It's frustrating losing your fish, especially when you don't know why.

Looks like it went in the filter and then was pushed out the opening. No way it could/would of swam into the outlet.

How would it get through the pump, though??

The thought did cross my mind @ 6am when I discovered it - but it would have been shredded and most likely would have stopped the pump...

Thats odd, it was moving fast obviously...

One would think it'd have to be moving quickly but clowns are generally only that fast when they're older - these are only about an inch long.
    Just baffles me.

I think Jason and Betty breed suicidal clowns LOL. Mine didn't do anything crazy like that one, but I did have a couple jump to their deaths.
